4D flow CMR detects progressive improvement in ventricular function following cardioversion of atrial fibrillation

چکیده
Background Atrial fibrillation (AF) causes impairment of cardiac hemodynamics and substantial cardiovascular morbidity. While AF is difficult to assess with cardiac gated CMR, successful electrical cardioversion of chronic AF is often followed by a transient period of atrial stunning where absence of mechanical atrial contraction, similar to that seen in AF, persists despite reinstitution of sinus rhythm. 4D flow CMR enables assessment of ventricular function according to the volume and kinetic energy of different LV flow components. The volume and end-diastolic kinetic energy (KE) of LV inflow passing directly to ensuing outflow (Direct flow) reflect aspects of left atrial-ventricular coupling and have been proven to be markers of LV dysfunction in failing hearts. In this study we hypothesize that left atrial (LA) stunning will contribute to impaired LV function reflected by reduced volume and end-diastolic KE of the LV Direct flow component.

Left Atrial 4D Blood Flow Dynamics and Hemostasis following Electrical Cardioversion of Atrial Fibrillation
Background: Electrical cardioversion in patients with atrial fibrillation is followed by a transiently impaired atrial mechanical function, termed atrial stunning. During atrial stunning, a retained risk of left atrial thrombus formation exists, which may be attributed to abnormal left atrial blood flow patterns. 4D Flow cardiovascular magnetic resonance (CMR) enables blood flow assessment from...

OBJECTIVES The purpose of this study was to evaluate left atrial mechanical function recovery and plasma atrial natriuretic peptide (ANP) release following successful cardioversion of persistent atrial fibrillation (AF). BACKGROUND Atrial fibrillation is characterized by functional deterioration, loss of atrial contraction, and elevation of plasma ANP levels. The response of ANP release towar...

OBJECTIVE To assess the clinical characteristics of patients in whom cardiac function improved after cardioversion of atrial fibrillation and the time course of the improvement. DESIGN A prospective serial study of echocardiograms recorded before cardioversion and one day, seven days, one month, and three months after cardioversion. SETTING Echocardiography laboratory of a university hospit...
متن کاملLeft ventricular diastolic function after electrical cardioversion of atrial fibrillation.
In chronic atrial fibrillation (AF) left ventricular function may gradually diminish as a result of an intrinsic tachycardiomyopathy. This tachycardia induced cardiomyopathy is characterised by decreased systolic function, ventricular dilation, and raised ventricular filling pressures. In such patients left ventricular systolic function may gradually improve after electrical cardioversion. Litt...

OBJECTIVES We sought to evaluate the effect of clinical factors on recovery of atrial function after cardioversion for atrial fibrillation. BACKGROUND Lack of effective mechanical atrial function (EMAF) after cardioversion of atrial fibrillation predisposes to thromboembolic complications and delays improvement in functional capacity. METHODS Fifty-two patients underwent cardioversion (grou...
